(1) Written application filed by an enterprise.

(2) Duplicate copy (with a seal affixed by the administrative departments for industry and commerce) of the counterpart of Business License of Enterprise Legal Person that has gone through the annual inspection.

(3) Duplicate copy of Tax Registration Certificate that has gone through the verification on a year basis.

(4) Duplicate copy of identification card of the legal representative as registered in the Business License of Enterprise Legal Person.

(5) Other materials necessary for declaration.
