(4) Documents required for applying for revocation of resident representative offices:

Such application shall include the following contents:
1) The time of establishment of the resident representative office and the approving documents.
2) The operation of the resident representative office after its establishment, including the issues as to whether it has gone through tax registration and whether it has ever applied to the customs for import of office supplies for itself in the name of the resident representative office.
3) Reasons for revocation of the resident representative office.

Notice:
1) A foreign enterprise shall submit its application for revocation of its resident representative office 30 days prior to the termination of its business activities. If a foreign enterprise makes such application more than one year after the expiration of its resident representative office, such representative office shall be deemed to have been cancelled automatically and the examination and approval authority may not issue the certificate approving its revocation.
2) The application shall be printed on letter papers with the name of applying enterprise as the letter head.
3) The name of the receiver of the application shall be "The Department of Foreign Trade and Economic Cooperation of Guangdong Province".
4) The application shall be signed by the chairman of the board of directors or general manager of the applying enterprise and shall be stamped with the common seal of such enterprise.

2 One duplicate of certificate of approval and one duplicate of registration certificate 1) The duplicate of certificate of approval issued by the examination and approval authorities for establishment or extension of the resident representative office.
2) The duplicate of the registration certificate issued by the registration authority (industry and commerce administration).
3 completed Application Form for Alteration of Resident Representative Offices of Enterprises from Hong Kong, Macao, Taiwan and Foreign Countries (in duplicate) 1) Such form is available at authorized agencies.
2) Such form shall be filled in by applying enterprises faithfully in detail and in the blank for alteration contents, "application for revocation" shall be filled in.
4 Procedures to be handled after the application for revocation is approved After the examination and approval authority approves the revocation of a resident representative office of a foreign enterprise, such enterprise shall go through the formalities with its bank of deposit, the customs, bureau of national taxation and bureau of local taxation on the strength of the certificate of approval and relevant documents. Then the enterprise shall go through the formalities of cancellation with Guangzhou Municipal Administration of Industry and Commerce on the strength of the certificate of approval, the certificates of cancellation issued by its bank of deposit, the customs, bureau of national taxation and bureau of local taxation, industrial and commercial registration certificate, and certificate of representatives. Finally, the enterprise shall go through the write-off formalities of code certificate with Guangzhou Bureau of Metrology on the strength of the certificate of revocation of industrial and commercial registration certificate.

Notes:
1. The examination and approval authorities shall have the right to require an applying enterprise to present notarial document in proof of the authenticity of those materials the enterprise submitted.
2. Where the above-mentioned materials are made in foreign languages, the Chinese text shall be attached thereto.
